About the Role:
At Apex Professional Care, our mission is to provide high-quality, compassionate care to individuals in their homes. As a Compassionate Home Care Specialist, you will assist patients with daily activities, providing essential support under the supervision of a Registered Nurse.
Key Responsibilities:
- Assist with personal care, mobility, and light housekeeping.
- Support patients with self-administered medication and therapy exercises.
- Monitor and report changes in patient conditions.
- Maintain accurate care records.
- Participate in ongoing training.

- Compassionate, reliable, and empathetic caregiver.
- Strong communication skills for building trust with patients and families.
- Physical and emotional resilience to meet caregiving demands.
- Training or competency in line with Medicare guidelines.
Required Qualifications:
- Certified Nursing Assistant (CNA)
Preferred Qualifications:
Basic Life Support (BLS) or Patient Care Technician (PCT) certification
Work Environment:
Patient homes, with travel required.
Physical Demands:
Requires physical tasks such as lifting, bending, and standing for long periods, with potential exposure to infection risks.
